Medication Therapy Optimization

Medications can be tricky to deal with. What you do not need anymore may carry over and over, while what you do need may not be prescribed. The dose may be too high or too small. It may treat one condition but worsen another. What you should take in the morning may be harmful if you took it in the evening. Medications may interact with each other, or with food, and so on, and so forth.
Doctors may not always keep up with the current flood of drug information, and the time constraints of your visit may not allow for a thorough medication therapy assessment; physicians rely on the pharmacist in that. At Entier Therapy, an experienced clinical pharmacist – the hospital doctors’ right hand – will review your medications in relation to your medical conditions, bloodwork, imaging results, blood pressure, heart rate, and more.
Examples of what can be done are assessing the desired effect and the side effects, adjusting the dosage or dosing time, stopping what may hurt rather than help, screening for interactions between drugs, or between a drug and a disease, or screening for missing treatment. Learning about your mood, sleep, appetite, stress level and lifestyle would help us see a comprehensive picture, necessary to approach your therapy properly.
How it works:
For assessment of medication therapy, we would need to know of your medical conditions and allergies, and to learn about your sleep, bowel function, appetite, energy level and mood. Additionally, please have the following information provided (as possible):
- The list of your current and recent medications;
- Recent bloodwork results;
- Current blood pressure, heart rate and blood sugar readings;
- Any investigations reports, hospital discharge summaries, or any doctor’s notes you were recently handed.
You will be asked to forward the information to us a few days before your appointment. During the appointment, we will review your therapy together. You may be asked a number of questions (please only answer if you feel comfortable to).
You will be provided with notes from the appointment and with recommendations to your doctor. If you wish, the recommendations could be forwarded to your doctor on your behalf.
Should you need further support, a follow-up appointment would be arranged for.
